About this directory and the Hawaii dermatology market
Step 1. Step 2. Match. Boards. Then what?
Then PGY-Final arrives. Suddenly there is no syllabus, no advisor telling you which clause to push back on, no upperclassman who has done this before. There is only a contract and a clock.
What this directory does is hand you the page you were missing. Every physician-owned dermatology group in Hawaii, in one place, so the first contract you sign is not the only contract you ever saw.
What other resources point to in Hawaii
You're looking at 65 physician-owned dermatology practices in Hawaii. I sourced all of them from the NPPES NPI Registry and filtered out everything that doesn't meet the standard of independent physician ownership. Hospital systems, academic centers, staffing agencies, and corporate-acquired practices are not on this list. Each entry includes the practice address, phone, website, and NPI number. The coverage spans 13 cities statewide.
Here's why I built this. When I was looking at private practice opportunities, I couldn't find a single resource that separated physician-owned practices from everything else. Job boards don't do it. Recruiter databases don't do it. So I built it from the NPPES registry. Whether you're finishing training, thinking about a lateral move, or exploring partnership-track positions in Hawaii, this gives you a filtered starting point that actually tells you who owns what.
The Hawaii dermatology market in detail
Hawaii's physician-owned dermatology directory lists 65 practice locations across 13 cities.
None of the dermatology practices listed in Hawaii carry a PE-Backed or Corporate-Acquired flag. All listings in this directory are physician-owned independent practices verified through NPPES.
Practices span 13 cities including Aiea, Ewa Beach, Haleiwa, and Hilo.

Frequently asked questions about dermatology in Hawaii
How many Dermatology private practices are in Hawaii?
There are 65 physician-owned dermatology private practices in Hawaii, verified against the NPPES NPI Registry. All are independently operated. Hospital-employed, government, corporate, and academic positions are excluded.
What does physician-owned mean?
A physician-owned practice is one where licensed physicians hold controlling ownership of the business entity. This is distinct from hospital employment, corporate medical groups, or private equity-backed practices where non-physician entities control the business.
What does the Corporate-Acquired or MSO-Affiliated flag mean?
The Corporate-Acquired flag means the practice was historically physician-owned but has since been acquired by a corporate entity such as Optum. The MSO-Affiliated flag means the practice uses a Management Services Organization for billing and administration while physicians retain practice-level ownership.
Cash-Pay and Cosmetic Models in Hawaii Dermatology
Hawaii has 7 physician-owned dermatology practices in this directory, including Hawaii Premier Dermatology (Urban Honolulu), Matsuda Dermatology (Urban Honolulu), and Sakamoto Dermatology (Urban Honolulu).
What is an NPPES-verified practice?
NPPES is the National Plan and Provider Enumeration System, a federal database maintained by CMS. Every practice in this directory has an active NPI record in NPPES, confirming it is a registered U.S. medical practice.
How do private dermatology practices in Hawaii structure cosmetic service revenue?
Cosmetic dermatology services (Botox, fillers, laser, chemical peels) are not covered by insurance and are billed directly to patients at market rates. Private dermatology practices in Hawaii typically keep cosmetic revenue separate from insurance collections. Some practices distribute cosmetic revenue equally among physicians; others let it accrue to the physician who performed the service. Ask how cosmetic revenue is split before joining and whether associates are permitted to build a cosmetic patient panel from day one.

Key Terms in Physician Contracts
NPI (National Provider Identifier)
A unique 10-digit number assigned to every U.S. healthcare provider and organization by CMS. Individual physicians receive a Type 1 NPI; practice entities receive a Type 2 NPI. The NPI is used across all payers for billing and credentialing.
Restrictive Covenant
Any contractual clause that limits a physician's professional activities after leaving an employer. Non-compete clauses are the most common restrictive covenant, but the category also includes non-solicitation clauses (prohibiting contact with former patients or staff) and non-disparagement clauses.
Fair Market Value
The price at which a willing buyer and willing seller would agree to transact, with both parties having reasonable knowledge of the relevant facts. Physician compensation must meet fair market value to comply with federal anti-kickback and Stark Law requirements. Independent valuations are used for buy-in pricing, compensation benchmarking, and practice sales.
Collections-Based Compensation
A pay model in which a physician's income is tied directly to the revenue collected from their patient encounters. The physician typically receives a percentage of collections (40 to 55 percent is common) after overhead. This model rewards high-volume, high-acuity work but creates income variability.
Malpractice Insurance
Professional liability coverage that protects physicians against claims of medical negligence. Two main types exist: claims-made (covers claims only if both the incident and filing occur during the active policy) and occurrence-based (covers any incident during the policy period, regardless of when the claim is filed). Premium costs vary dramatically by specialty and state.
Direct-Pay Practice
A medical practice model in which the physician does not contract with any insurance payer and collects all fees directly from patients. Eliminates billing overhead, prior authorizations, and claim denials. Common in concierge primary care and cosmetic dermatology. Patients pay out-of-pocket; some may be reimbursed by their own insurance.
Cosmetic vs. Medical Dermatology
Medical dermatology encompasses diagnosis and treatment of skin disease (acne, eczema, psoriasis, skin cancer) billed through insurance. Cosmetic dermatology includes aesthetic procedures (Botox, fillers, laser resurfacing) paid directly by patients. Practices with both revenue streams are typically more financially resilient than those relying solely on insurance billing.
